I agree with Carli that it depends on the state you are in. If there is a book on the subject call Beth Jezek at PMIC and ask her, she is a wealth of information. 1-800-633-7467 x547 (Tell her you are from the Medical Association of Billers-to get a discount)
If you post your state, as Carli suggested, we will see what numbers we have for you. I would start with the state Welfare Office, State Division for Insurance, then the State Health Insurance Information Program (SHIP).
