I have just begun working with an Emergency Medicine physician group and they are having some concerns regarding reimbursement. Their per patient reimbursement has gone down in 1 year by $4.00 per patient. The payor mix is the same, the contracts and fee schedules are the same or up, the acuity has gone up and a few new services have been added that should increase revenue. They are working with an out of state billing company. I have recommended a coding audit, documentation workshops for the physicians (due to a disparity in documentation from physician to physician), a close review of denials, and an audit of received payments versus agreed upon fee schedules. My background is in healthcare management, but not in the physician billing side---so, I am a little unclear as to how the aging process works and how it should look. When accounts are older than 180 days---is there a formula of how this should look in percentage of receivables? Is there any formula that would tell me how much should be this old? For instance---10% of receivables, more/less? Also, when is it preferable to send an account to a collection agency? Is 90 days too soon? What is a good turnaround time? Does anyone know what an ED group should be receiving per patient? $60.00, more? less? I have been searching for some books on managing receivables so I could become more familiar, but have not found much out there. Any suggestions? Any help you can provide would be very appreciated. Hi Cherie,

I've been billing for psychiatrists for the past 4 years. From my experience, I have never used 90804 or 90808. Here are the codes that I always use: 90801: Initial evaluation 90806: Individual therapy which used to be 90844 90847: Family therapy 90853: testing 90862: Medication management If your claim was to medicare, the codes are different on that. You have to know, the business and the specialty very well. Docs also usually work with someone they know or have worked with in the past. Its also best if you located docs that are solo and need the help. They are usally understaffed and hungry. Be creative. Start off by consulting and gaining their confidence. Evaluate their practice and "show them" they need you and what are the savings of letting someone else do their billing. Docs are stubborn. Be tenacious.

I am looking into becoming a medical biller and answered a newspaper ad seeking medical billers. It was Medco and they say they provide you with physicians to do billing with. Does anyone have experience with this company and with Medisoft (their software)? Please respond with any advice. Already checked them out with BBB (Better Business Bureau) and nothing was reported. I am new to the field of Pediatrics. I need to know if there any any Administration codes that can be used to bill for things like syringes used for injections. I also need to know if a well-visit can be billed under any other code so that a person who has reached a $200.00 max for well-visit check-ups does not have to pay out of pocket. I have recently started a medical billing business I am having a problem on getting any doctors to take on my services. In fact I have not heard from. I would like to know if it is hard to run a medical billing business. The company I bought the program said I would indeed hear from physicans.
